NDIS Support Coordination
Innovate Support Coordination is a registered NDIS Provider (Provider Number 4050085326) based in Wagga Wagga servicing the needs of NDIS participants throughout the Riverina, we service our clients by connecting them to local services that meet their needs, connecting them to their community and enabling them to live a life of independence and choice.
A Support Coordinator may undertake the following tasks
support a participant to understand how they can use their NDIS plan to meet their goals;
support a participant to effectively manage those resources to get the best outcome from their plan;
connect a participant with services and supports that met the participants goals;
support the participant in exercising choice and control over their funding;
work with other people in the participants circle of support (family, friends, service providers, therapists and community and mainstream supports such as doctors) to meet the goals of the participants NDIS plan;
support participants access to the NDIS portal to monitor how their funds are being spent
monitor the progress and outcomes of achieving a participant’s goals;
assist a participant to prepare for a NDIS review meeting; and attend an NDIS review meeting at the participants request.
